Vision services must be approved by SafeGuard Vision Plan.

This vision plan provides benefits that will allow your child to receive excellent vision care. When visiting a SafeGuard network doctor, your child will receive:

     • Exam (Every 12 months)
     • Prescription Glasses Lenses (Every 12 months)
       Single Vision; Lined bifocal; Lined trifocal lenses
     • Frames (Every 24 months)
       Frame of your choice covered up to $115.
       Plus, 20% off any out-of-pocket costs.
